Final Report Forms

​​​​​​​​Organizations and individuals that receive funding from the Wisconsin Arts Board are required to file a final report for the year's activities. Deadlines vary for each program.

Final report forms may be accessed through the links below. Grant recipients are asked to fill out and submit the form online, then print, sign, and mail the form to the Wisconsin Arts Board, PO Box 8690, Madison, WI 53708 - postmarked no later than the program's deadline date. Your final report will not be complete until we receive the signed paper copy. Please retain a copy for your files.

Applications from organizations that have not filed final reports for past grants will not be accepted for panel review. For this reason, final report forms for prior fiscal years are also posted below.

After submitting the form you must print it a) to keep a copy for your records and b) to send to the Arts Board. Instructions are contained in each form.
